''Duke Nukem: Manhattan Project'' byl navržen na základě 3D prostředí známého jako [[Prism3D]]. Úrovně a postavy jsou plně trojrozměrné a kamera i Duke se může pohybovat po kterékoliv ose, ale pohyb je omezen dvojrozměrnou rovinou. Díky 3D enginu může hráč přibližovat a oddalovat kameru na přicházející nepřátele nebo na zobrazení celého bojiště. Duke se umí skrčit, běhat, skákat a skluzem se umí protáhnout pod překážkami.
 
Hra je rozdělena do osmi kapitol, ž nichž má každá tři části. V každé části musí hráč osvobodit dívku připoutanou ke GLOPP bombě a najít barevnou přístupovou kartu potřebnou na odemčení další části. V některých částech dostane hráč možnost za pomocí [[Raketový batoh|jetpacku]] přeletět velké prázdné oblasti nebo nebezpečné úseky. Ovládání je také velice jednoduché na naučení, používají se pouze klávesy na skákání, pohyb, střelbu a změnu braně. Použitím cheatu lze natočit kameru do libovolného úhlu a pořídit si tak screenshot. Herní CD obsahuje level editor pojmenovaný ''PrismEd'', ale tvorba levelů nikdy nedosáhla přílišné popularity mezi hráči a dnes je aktivní pouze malá komunita editující levely.

V roce 1996, George Broussard byl dotazován na budoucnost projektů společnosti 3D Realms: řekl, že Duke Nukem plošinovka nazývána ''Duke Nukem Forever'' byla ve vývoji a měla by přijít na trh o Vánocích 1997. Projekt byl později zrušen a název ''[[Duke Nukem Forever]]'' znovu přiřazen pro opravdové pokračování ''Duke Nukem 3D''. In 2004 ''Manhattan Project'' developer [[ARUSH Entertainment]] was bought out by HIP Interactive. Soon afterwards, HIP went bankrupt, and took ARUSH down with them. Because of bankruptcy proceedings, the legal rights to ''Manhattan Project'' are now held by a court appointed bankruptcy firm. [[3D Realms]] has inquired about retrieving the rights, but has been unable to do so. Manhattan Project received mostly positive ratings, in the range of 7-8 out of 10. GameSpy gave the game 83%, calling it "A slick platform arcade game at a reasonable price... captures Duke perfectly; great system performance; clever use of 3D. Similarly, Game Over gave the game an 81% score, praising it: "Movement is kept at a brisk pace and there are plenty of monsters to blow away... a refreshing evolutionary step for the platform game without play devolving into a third person behind-the-back affair" GameSpot, with a 7.9 out of 10, was more neutral, saying, "It's straightforward and good-looking... the levels are huge, and most have several paths you can take."<ref>{{cite web|title=Duke Nukem: Manhattan Project (PC)|url=http://www.gamespot.com/pc/action/dukenukemmanhattanproject/index.html|publisher=Gamespot|accessdate=25 April 2011}}</ref> IGN awarded it a 7.7 out of 10, citing, "Manhattan Project is a polished, tried, and true title... worthy of a recommendation, especially given its keen price point and familiar antihero."
