«Прямоугольное полотнище жёлтого цвета с отношением ширины к длине 2:3, воспроизводящее композицию герба муниципального образования Сакмарский района в синем, чёрном, красном, зелёном и белом цветах».

Геральдическое описание герба: «В золотом поле оренбургский казак в форме и снаряжении конца XIX века (чёрной папахе с синим верхом, зелёной униформе с лазоревыми погонами и лампасами, чёрных сапогах, при шашке с золотой рукоятью, в чёрных отделанных золотом ножнах), держащий в руке красную пику с лазоревым наконечником и сине-серебряным значком (хорунжевкой), сидящий на чёрном идущем коне с червлёными упряжью и седлом, сопровождаемый во главе щита узким лазоревым стенозубчатым сверху поясом, а в оконечности — лазоревым же узким поясом, вверху выщербленным, внизу чешуйчатым».
